Jowls tend to arrive quietly. One day the jawline that used to look clean and defined has softened, and the lower face reads as tired even when you feel anything but. It is one of the most common concerns we hear about at MediZen, and the good news is that a great deal can be done before anyone starts talking about surgery. Our doctor-led team in Birmingham and Sutton Coldfield treats jowls every week, and this guide explains why they form and the non-surgical options that genuinely help. You can also see the full picture on our jowls treatment in Birmingham page.
What are jowls, exactly?
Jowls are the small pockets of soft tissue that form along the jawline, on either side of the chin, as the lower face loses its firm, clean edge. They are not a sign of weight gain or of doing anything wrong. They are simply what happens when the structures that hold the lower face taut begin to change, and they show up on slim and fuller faces alike.
Why do jowls form?
Jowls are really three separate problems wearing one name, and understanding which is driving yours is what points to the right treatment.
- Lost volume and support. From the mid-thirties the deep fat pads that hold the cheeks up begin to shrink and slip downward, so support that used to sit high on the face gathers along the jaw.
- Skin laxity. Collagen and elastin decline with age, so skin that once bounced back now drapes a little more loosely over the jawline.
- Muscle and bone change. The muscles of the lower face and subtle changes in the jawbone itself alter how everything sits.
Most people have a mix of all three, which is why a proper assessment matters more here than with almost any other concern.
Can you treat jowls without surgery?
For mild to moderate jowls, yes, and often very effectively. The trick is to treat the cause rather than chase the sag. Restore the lost support, improve the quality and firmness of the skin, and tighten what has loosened, and the jawline looks lifted and rested again without a single incision. Which combination is right depends entirely on your face, which is why every plan at MediZen starts with a face-to-face consultation.
The non-surgical treatments we use
Restoring structure. When lost volume is the main driver, carefully placed dermal fillers rebuild support higher on the face and along the jaw, so the jawline reads as defined again. This is doctor-led work at MediZen, led by our clinical director Dr David Eccleston, because the lower face is unforgiving of a heavy hand. The aim is a natural, refreshed jawline, never an overfilled one.
Improving skin quality. Where the skin itself has thinned and lost its bounce, Profhilo and other bio-remodelling treatments rehydrate the skin and stimulate your own collagen, so it looks firmer and more elastic over a short course.
Tightening what has loosened. For skin laxity, energy-based skin tightening such as Secret PRO radiofrequency microneedling heats the deeper layers to contract existing collagen and prompt new growth, gradually firming the jawline over the weeks that follow.
What to expect
Filler results are visible straight away and settle over a couple of weeks, while bio-remodelling and skin tightening build gradually as fresh collagen forms. Most of these treatments involve little to no downtime, so they fit around normal life. Your practitioner will set out an honest timeline at your consultation, including how many sessions you are likely to need and how long the result should hold.
When surgery is the better option
Honesty matters here. Non-surgical treatments do a great deal for early to moderate jowls, but they are not a facelift. If there is significant, heavy sagging, a surgical lift may give you a better and longer-lasting result, and we will tell you plainly if that is the case rather than sell you a plan that will not deliver. A consultation is the only way to know which camp you are in.
Frequently asked
Do fillers help jowls or make them worse? Placed correctly, filler restores the support that has been lost higher on the face, which lifts the jawline. Placed poorly or too heavily it can drag, which is exactly why jowl treatment should be doctor-led and conservative.
How long do the results last? It depends on the treatment. Filler results typically last many months to a year or more, while skin-tightening and bio-remodelling results are maintained with occasional top-ups. Your practitioner will map out a maintenance plan that suits you.
